After reviewing the Berkeley Film Guidelines, please download and complete the appropriate documents below and email them to BOTH [email protected] & [email protected]. A Film Office representative will contact you once your request has been reviewed.

1. Download all forms to computer desktop.
2. Open each Adobe PDF document, complete, and save to computer.
